Your provider can call your prescription into The Apothecary Shoppe Pharmacy for you to pick up, or you can take it yourself! When you bring or pick up your prescription, be sure to bring the following:

• Your current insurance card and information
• Your driver’s license or government issued id

• The Patient’s Name
• Equipment Ordered (if any)
• Quantity Ordered
• Length Of Need
• A Qualifying Diagnosis
• The Physician’s Signature
• The Date The Prescription Was Written
