With a qualifying prescription, The Apothecary Shoppe Pharmacy will be glad to bill your insurance company, Medicaid or Medicare! Qualifying prescriptions must include the following:

• Equipment Ordered (if any)
• Quantity Ordered
• Length Of Need
• A Qualifying Diagnosis
• The Physician’s Signature
• The Date The Prescription Was Written
